Submission Guidelines
1. :26-:28 PSA in an acceptable format.
2. Completed entry form (on line) at www.healthflix.org.
3. A video/photograph release form for each person under age 18 appearing in the video signed by his/her guardian. This release form must be faxed to (937) 645-3047 or emailed to contact@uchd.net.
4. For individuals and teams less than 18 years of age or entering under the Middle School or High School Groups, entries must identify a faculty advisor and school to which at least one member of the creative team belongs. All HealthFlix entries must be received by 11:59 p.m. EST on Tuesday, May 01, 2009 (including entries submitted by postal carrier). Entries after that time will not be accepted.
Format
The PSA must be submitted as a full quality/full frame (720 X 480 pixel) QuickTime, AVI, WMV, or MPEG video no longer than 28 seconds in length. A video that is 26-28 seconds in duration to be tagged with a health message or statement to create a 30 second spot that could be aired on TV.
If submitted by standard mail, the entry should be on CD in AVI format. Be sure to label your entry with the advisor’s contact name, school name, organization/student, and project title. Entries will not be returned. VHS or other media not identified elsewhere in this document will not be accepted.
Submitting an Entry
Entries can be submitted online or mailed as a CD before the deadline. However, uploading is strongly encouraged over regular mail in order to guarantee a timely submission.
Videos must be uploaded or received on or before 11:59 p.m. EST on May 1, 2009.
Uploading
1. Upload video to www.youtube.com. The video must be 26-28 seconds long and less than 10 MB in size.
2. On YouTube, Tag your video with healthflix (one word, all lowercase).
3. Go to www.healthflix.org to complete your registration.

If a submission cannot be uploaded, subnit your entry on CD in AVI format to:

The CD must be identified with the name of the team captain, the topic category and age group the entry belongs to, and include a completed entry and release forms available at www.healthflix.org.
